Two possible options:
1. Submit via le "local commune": you may submit your LAMal exemption request through your local commune. Please include the following documents:
- Exemption form request provided by LAMal duly completed
- A copy of your Swiss legitimation card, as holding a legitimation card exempts you from LAMal affiliation
- A printout of the relevant webpage on health insurance from the Swiss Mission's website
- A printout of the Allianz webpage to inform the LAMal Service your intention to subscribe to Allianz
2. Submit directly to LAMal Services:
- Alternatively, you may send your exemption request directly to the LAMal Services, including the same supporting documents listed in option 1.
- Address: Service de L'Assurance Maladie - Route de Frontenex 62 - 1207 Genève
You may use the CERN Hostel's address in the subscription form.
Please visit the UNIQA office at CERN for more information.
Persons enjoying privileges and immunities are not required to take out health insurance, but they may take out Swiss health insurance on a voluntary basis and subject to conditions. Family members, holders of a type "P" legitimation card, are also not required to take out health health insurance. Family members, holders of a type "H" legitimation card, are however not exempted and are subject to Swiss health insurance.
Please refer to the Swiss Mission's manual on Health insurance for full details.

ACCU presentation (3 December.2019)
An A1 certificate is required for Users who are employed by a home institute in the EU or in an EFTA country and travel to another EU/EFTA country for business.
The Home Institution Declaration by which the User’s home institution declares that they have adequate social security coverage in Switzerland, France, or other countries visited on duty travel, is an internal means for CERN to ensure that Users remain constantly covered, but the HID cannot be seen as material proof of such adequate coverage or as an exemption from the A1 requirement. The HID therefore does not replace the A1 certificate.
Although users are not requested to show their A1 Form to the Users Office upon registration or contract extension, they must carry an A1 Form when traveling to CERN.
