What is the procedure to terminate my affiliation with LAMal before subscribing to the Allianz Healthcare Plan for CERN MPA given that Allianz will not sign the exemption form?

Two possible options:

1. Submit via le "local commune": you may submit your LAMal exemption request through your local commune. Please include the following documents:

Exemption form request provided by LAMal duly completed
A copy of your Swiss legitimation card, as holding a legitimation card exempts you from LAMal affiliation
A printout of the relevant webpage on health insurance from the Swiss Mission's website
A printout of the Allianz webpage to inform the LAMal Service your intention to subscribe to Allianz

2. Submit directly to LAMal Services:

Alternatively, you may send your exemption request directly to the LAMal Services, including the same supporting documents listed in option 1.

Administrative matters

Departmental Administrative Officers (DAOs)

DAOs handle other formalities for COAS and VISCs, such as COLAs, subsistence allowances, invitation letters and conventions d’accueil, allocation of phone numbers and internal addresses, duty travel, etc.

Experiment Secretariats

In addition to space management, event management and membership database management, Experiment Secretariats take care of the registration of External Participants in an Experiment, access to experiment data, invitation letters and conventions d’accueil, allocation of phone numbers and internal addresses, duty travel, subsistence and cost of living allowances, etc.

Human Resources

The CERN Human Resources Department handles matters related to careers as well as financial and social benefits for all categories of personnel (except Users, COAS and VISCs).

Users Office

The Users Office handles all the formalities relative to contracts of association and Host-State documents (Swiss and French work and residence permits) for Users, COAS and VISCs, and provides general information and support.
